Abstract


American healthcare is in crisis — costs are out of control and quality is deteriorating. The solution is for all stakeholders (patients, providers, researchers, purchasers, insurers, public health agencies, etc.) to participate in revamping our healthcare system and building and using evolving health science knowledgebases and health information technology (HIT) that enable the economical delivery of high-quality (safe, effective, efficient, affordable, timely, and available) care, preventative services, and biosurveillance for all people. This is the essence of our Patient Lifecycle Wellness model and Quality through Knowledge strategy — a wise solution that focuses on:
  • Establishing collaborative networks of stakeholders that develop, share, analyze, and discuss health information and innovative ideas resulting in the implementation of evidence-based practice guidelines and ongoing clinical research.
  • Developing, using, and refining economical HIT tools for all stakeholders guided by their ongoing feedback, so the tools are useful, practical, and help promote continuously improving health outcomes and lowering costs. These tools and the data they manage must be highly secure, survivable and accessible during a crisis, and enable in-depth understanding of each patient’s health problems and best intervention options.
  • Using this comprehensive knowledge of the person, together with complete knowledge of the scientific research and the experiences of providers in everyday practice to (1) make the best possible treatment decisions within a “personalized care” framework that tailors interventions to the specific needs and conditions of the individual patient, (2) deliver that care efficiently and effectively, and (3) enable all consumers to be competent, proactive, informed participants in the healthcare decision process and in promoting their own health by educating them about treatment and provider alternatives, by monitoring their lifestyle changes, and by giving them feedback about how their affects their health.
  • Improving the “fidelity” of care by making changes in the healthcare system to allow all patients to receive the precise interventions they need, properly delivered at the time they are needed.
  • Strengthening fiscal cost-control strategies by assuring that care quality improvement is a fundamental component of all proposed solutions to our healthcare crisis.
